I can’t offer any technical advice on this, but I feel I should mention that what you have there is not a mobile design, but rather a web-centric design. The whole mechanic displayed will fall apart pretty quickly if you try to adapt it to a mobile use-case. For example, the whole card is hovering over a background. On mobile this would most likely not happen since you’d use the full width of the mobile phone screen for forms like this.
You can take a look at one of our existing examples that displays pretty much the same mechanic as shown in your inspiration above.
